I was prescribed 0.5mg alprazolam as-needed for panic disorder after trying SSRIs and therapy. My psychiatrist was very clear: this is a rescue medication, not a daily one. Use it when a panic attack breaks through, maximum twice a week.
T+0:00 placed the tablet under my tongue during a panic attack that hit me while working from home. Heart pounding, chest tight, convinced I was dying — the usual panic attack cocktail. T+0:15 the first wave of relief. The physical tension in my chest loosened. My heart rate started coming down. T+0:25 I could breathe normally again.
The effect is not euphoric at this dose. It's the absence of terror. Like someone turned off an alarm that was blaring in my brain. I felt calm, slightly drowsy, and profoundly grateful. I finished my work day and ate dinner normally.
I use it maybe 3-4 times per month. I've never increased the dose. It works exactly as intended when used as a tool rather than a crutch. The key is having it available — just knowing it's in my bag reduces panic attacks because I'm less afraid of having one.
