I was diagnosed with prediabetes at 42. Fasting glucose of 118, HbA1c of 5.9%, triglycerides at 210, LDL at 158. My doctor wanted to start metformin. I asked for three months to try lifestyle changes first — diet modification, walking 30 minutes daily, and supplements. She agreed.
Among the changes, I added black seed oil (500mg capsules, 4 per day with meals) based on the meta-analyses showing modest but statistically significant improvements in fasting glucose and lipid panels. I did not take it in isolation — I also cut refined carbohydrates, added more fiber, and was consistent with the walking.
Week 4 home glucose monitoring: Fasting glucose readings were trending down, averaging 106 vs my baseline of 115-120. I also noticed I was not getting the post-meal drowsiness that used to flatten me after lunch. My energy was more stable throughout the day.
Week 8: My home readings were consistently in the 98-105 range. I was also down 8 pounds from the dietary changes, which obviously contributes. But the glucose improvement seemed disproportionate to the weight loss.
Month 3 lab results: Fasting glucose 98 (down from 118). HbA1c 5.5% (down from 5.9%). Triglycerides 156 (down from 210). LDL 132 (down from 158). HDL went from 42 to 48.
My doctor was impressed and agreed to hold off on metformin. She was honest that the improvements were likely multifactorial — diet, exercise, and weight loss were doing heavy lifting — but she acknowledged that the black seed oil may have contributed to the glucose and lipid improvements, citing the clinical trial data.
I cannot isolate how much of this is black seed oil versus the other lifestyle changes. But the clinical evidence supports it having a real, if modest, effect on metabolic markers. I continue to take it as part of my daily supplement regimen. The 2g daily dose in capsule form is easy and has no side effects for me.
This is not a replacement for metformin or other medication if your doctor says you need it. But as an adjunct to genuine lifestyle modification, it appears to offer meaningful metabolic support.
