I want to give an honest, measured account because I think the BPC-157 community can lean toward overly enthusiastic reporting.
I had a partial thickness supraspinatus tear with associated tendinopathy, confirmed by MRI. Orthopedic surgeon said it was borderline for surgery -- could try conservative management first. I was doing PT three times per week and decided to add BPC-157 at 300 mcg subcutaneously near the anterior deltoid, twice daily for 8 weeks.
The results were real but not miraculous. Pain during overhead reaching decreased from maybe a 6/10 to a 3-4/10 over the 8 weeks. My PT noticed that I was progressing through the strengthening exercises faster than she expected. Night pain (the worst part of rotator cuff issues -- that aching that wakes you up when you roll onto the affected shoulder) improved significantly by week 3.
However, I still have limitations. I cannot do a full overhead press. I still get pain at end-range external rotation. The MRI showed improvement in the tendon appearance at my 3-month follow-up, but the tear is still present, just smaller.
My honest assessment: BPC-157 meaningfully accelerated my recovery and reduced my pain, but it did not work the miracle cure that some internet accounts would have you believe. For a partial tear, it was a valuable addition to physical therapy. I suspect it would be even more effective for pure tendinopathy without a structural tear.
One note on injection site: I initially injected in abdominal fat but switched to injecting near the shoulder per community recommendations about injecting close to the injury. I felt like the shoulder-proximal injections were more effective, though this could be placebo.
