I have been using medical cannabis for chronic lower back pain for about three years. My typical dose is 20-25mg THC in edible form, which provides good pain relief for approximately 4-6 hours. I heard about THC-O being more potent and wondered if a lower milligram dose could provide equivalent relief with a longer duration.
I obtained a third-party tested tincture and began with 5mg oral doses taken in the evening. Observations over 10 days:
Day 1: Onset at approximately 50 minutes. Noticeable pain relief beginning around T+1:15. The quality of the analgesia was similar to my standard THC edibles — not a complete elimination of pain, but a significant reduction in the sharpness and intrusiveness of it. The relief lasted approximately 5-6 hours, which is comparable to my usual 20-25mg THC dose. The psychoactive effects were mild but present: light euphoria, slight sedation, typical cannabis headspace. Less impairing than my usual dose.
Days 2-5: Consistent onset times, consistent pain relief duration. I noticed the sedative component was more pronounced than with my regular edibles — this was actually beneficial as I take my dose in the evening and the sedation promoted better sleep, which in turn improved pain levels the following day.
Days 6-8: Some tolerance development to the euphoric effects, but pain relief remained consistent. This mirrors my experience with regular THC — the recreational effects diminish faster than the therapeutic ones.
Days 9-10: Pain relief still present but slightly reduced. I would expect to need a tolerance break within another week, similar to my pattern with regular THC.
Overall: 5mg THC-O provided comparable pain relief to 20-25mg regular THC edible, supporting the 2-3x potency estimate. The duration was similar or slightly longer. The lower milligram dose meant less intense psychoactive effects, which was actually preferable for functional evening use. The main drawback is the complete absence of regulation, testing standards, or medical guidance for dosing. I am essentially running my own uncontrolled pharmacological experiment, and I am aware of how uncomfortable that should make me.
